In applying for a competitive scholarship from my school district to attend a program at Herndon Career Center or Southland CAPS for the 2024-25 school year, I agree to the following guidelines:

1. I agree to commit one to two hours outside of class for each day spent in class. Career & Technical Education programs require students to be fully prepared for class including studying for tests, completing homework, advance reading, concept reviews, etc.

2. I will maintain a 90% attendance record. In the event that my attendance drops below that level during a semester of attendance, I will be placed on an attendance plan. If I do not meet the expectations of the attendance plan, I will be dropped at semester and/or not be eligible to return to HCC/CAPS. Students who miss 10 consecutive days will be dropped.

3. I will maintain an acceptable conduct record at both HCC/CAPS and my sending school. I understand that suspensions from either HCC/CAPS or my sending school may result in removal from HCC/CAPS. Safety is a top priority at HCC and Southland CAPS. Incidents that compromise the safety of students, staff, or equipment may result in immediate dismissal.

4. I will maintain a C or better grade in my classes at HCC/CAPS. Semester grades of D or below may result in removal from HCC/CAPS classes at the semester. I understand that enrollment in the second year of a two-year Center of Excellence program is contingent upon successful completion of the first year course with a C or better grade, have 90% attendance, a HCC/CAPS teacher and admin approval or as return requirements outlined in the Career Education Planning Guide.

5. While in attendance at HCC/CAPS, I will conform to the dress code in effect for the program in which I am enrolled. This will include any program-related work experiences in which I participate.

6. If removed from the program for any reason, I understand that I may lose credit for courses taken at HCC/CAPS and that I may not be able to enroll in coursework at my sending school until the next enrollment period. I understand that a semester program earns 1.5 credits; a year-long program earns 3 credits, except for Cosmetology which earns 8 credits per year.

7. By signing this form, I allow my school records to be released to Herndon Career Center or Southland CAPS for inclusion in my HCC/CAPS educational file.

8. I understand that if I am in: Cosmetology, Foundations of Nursing or any Southland CAPS program that providing my own transportation to/from is a requirement for all or parts of the school year.

9. Your application will be assessed by your school on the basis of the following: program prerequisites, interest in a program as evidenced by this application, student essay and interest inventory assessment, recommended coursework, and the number of high school credits earned to date. The final decision for program placement rests with the site director.

10. Students must be endorsed by their sending high school/district to attend HCC/SC. Students must maintain academic progress and remain in good behavioral standing in order to attend HCC/SC. Students who are long term suspended are not eligible to apply or attend HCC/SC.
