As parent/legal guardian of the above player, I hereby give the consent and approval to participate in the above named activity during the current season. I certify that my child is in good health and able to participate in all activities. I hereby release the California Grapettes (CG) from any and all claims and liability of any kind of personal injury or property damage due to participation in this program. I assume all risk and hazards incidental to such participation in any/all activities and hereby waive, release and agree to hold harmless CG, its board of Directors, managers, coaches and volunteer participant, from any claim arising from injury to the above player.
