Welcome to the Gluck Session Evaluation Form. We hope this option provides a convenient method for tracking our Fellows' impact in the community. Fill in requested information below.

The two Student Response questions at the bottom can be based on an in-class discussion you have after the Fellow leaves, or responses you have them write in an exercise. We only need one representative response that best sums up what the students thought about each question.
