BIOLOGY: Laboratory Safety Code of Conduct
The science courses at Upper Merion are hands-on laboratory courses. You will be doing many laboratory activities, which may require the use of hazardous chemicals. Safety in the science classroom is the #1 priority for students, teachers, and parents. To ensure a safe science classroom, a list of rules has been developed and provided to you in this student safety contract. These rules must be followed at all times. The Laboratory Safety Code of Conduct form must be signed by both the student and a parent / guardian before the student can participate in any laboratory investigation. Please let me know if you would like a paper copy of the Laboratory Safety Code of Conduct.

Please review the safety guidelines on the following pages by FRIDAY, September 8th.

Both the PARENT/GUARDIAN and STUDENT will be required to virtually sign this code of conduct at the end of this form.
