Please use this form if you have an issue or complaint that has not been satisfactorily resolved after contacting your coach (for coach related issues) or your team manager, MVLA Board or staff member (for other issues). The Escalation Committee will forward your issue/complaint to the appropriate MVLA staff member and someone will get back to you within 48 - 72 hours of submission.
Please review MVLA's Communication Policy before submitting this form. For example, for all coach related issues, it is vital that you contact the coach first to give him/her an opportunity to resolve the issue or address the complaint. Thank you.
