Please fill in this form if you would like to request anything from 3dtoyshop. This generates your request ID, and gives you access to the teddy hospital portal https://teddyhospital.azurewebsites.net/ where you can upload pictures of devices and scars, add notes to your request (in case of changed medical conditions) and pay for your teddy (if you select prepay).

This part is very important: The only things the teddy surgeons who make your teddy will see is the information entered into this form, and the information entered into teddy hospital portal when its time to order your bear. Whatsapp messages, facebook messages, tiktok comments, emails, none of these are printed on the form used by the teddy surgeons to make your teddy.

You will get an email from admin@3dtoy.shop when it is time to order your bear. Please add this to your email contacts so it doesnt go to spam. Your teddy will only cost £20 plus shipping (which is £5 to the UK or £20 abroad). And you will not be able to pay for your bear before you have reached the front of the request queue.

If you are unable to pay for your teddy, you can apply to the sponsor a teddy scheme scheme. These teddies are free of charge and are funded by donations to the go fund me. Please note, applying for this scheme does not guarantee a teddy, we have to pick between the children in the most need and the money available in the fund. https://www.gofundme.com/f/3dtoyshop-sponsor-a-teddy-scheme

Important Notes:

1) After filling in this form, you will not have immediate access to the portal. Nick has to manually register new users at present and this may take a month

2) Once nick has registered you with the portal, you will be able to add notes to your request, add pictures, but you will not be able to order your teddy as you are in the queue.

3) It is now possible to change your request from pre-pay to teddy scheme and vice versa within the portal.

3dtoyshop runs at a loss to help the little heroes, so donations are much appreciated.
