Head Mat and Pairing Officials Tournament Report
The report is in three sections. The first two must be completed by the Head Mat Official. The Head Pairing Official is asked to complete the all three (3) sections.
     *In the 1st part you will be asked to fill in the names and emails of the tournament leaders. It is important that the information in this section is accurate.  All information contained in the "Tournament Report" is passed on and shared so that tournament directors and leaders can use the information to improve the tournament experience for the athlete, coaches and volunteers involved in future USAW events.
   *The 2nd section is a general evaluation and report of the tournament.  
  * ALL RESPONSES must contain valid comments or information.  Simply click on the boxes and type in appropriate comments. KEEP ALL COMMENTS AND RESPONSES CONSTRUCTIVE AND PROFESSIONAL.
   * The 3rd and final section is information specific to Pairing officials

When completed the report is automatically saved when the SUBMIT button at the at the very end of the survey is clicked.
