Rising Sun's Mentorship Program - Mentor Form
Rising Sun's Mentor Program purpose is to mentor Black boys and youth ages 8 to 25 years old, from the African, Caribbean, and Black communities. Priority is given to boys and youth from single-parent homes, vulnerable, marginalized, at-risk youth and youth in conflict with the law. This form is the first initial step in the mentoring process. No information on this form will be shared with any outside organizations. Please complete the form below if interested in being a mentor. Someone from our team will be in contact with you. We ask that if persons interested in mentoring cannot commit approximately 5 hours a month that they do not complete this form. As the boys and young men in the program look forward to meeting consistently with their mentors on a regular basis. Inconsistencies can impact the lives of young people in negative ways, therefore, we are seeking persons who are and can be consistent.

Thank you for your interest in mentoring. The information collected on this form is the initial first stage of the application process for potential mentors. The program will run for one year and will be extended based on mentor availability.  All mentors will be required to attend leadership and mentoring training sessions prior to mentoring. All mentors are required to complete a Vulnerable Criminal Reference Check. Preference of mentors and mentees is given to Black males from the Black, African and Caribbean communities. Please note, within the Rising Sun's Mentor Program, we have a specific mandate, preferences is given to Black boys and youth from single-parent homes, underserved communities, vulnerable and at-risk youth, as well as youth in conflict with the law. However, all Black boys and youth that register for a mentor, will have access to a mentor. Person's interested in mentoring must be consistent. Please note, as Covid-19 Restrictions Lift, all mentors will be expected to meet in person with their mentees. These meetings maybe group meetings with everyone, or individual meetings. As well, mentors will attend a Mentor Meetup, where they meet all the other mentors. This provides the opportunity for mentors to network with each other. Are you able to meet in person. *
