I acknowledge that I am aware of the inherent risks of injury or health concerns that may arise from the participation of my child in the Kids and Teens Acro, Aerial Fitness, Pole Dancing, Lyra, Aerial Sling, Stretch, Dance classes, and all other kids classes offered by Wanderlust Pole Studio Pty Ltd located in Liverpool NSW Australia. I understand that these activities involve physical exertion and have potential risks associated with them.

I willingly choose to allow my child to take part in these activities and understand that they will do so at their own risk. Therefore, I hereby release and waive any claim against Wanderlust Pole Studio Pty Ltd, its owners, and their respective representatives, from any liability for injuries, illnesses, or any changes in medical condition or health status (whether temporary or permanent) that may directly or indirectly result from the use of Wanderlust Pole Studio's facilities or participation in its services, regardless of whether they are supervised by Wanderlust Pole Studio staff.

I grant my consent for my child to receive necessary medical or ambulance assistance in case of injury or emergency while participating in these classes, and I agree to assume responsibility for any associated costs incurred.

I confirm that the information provided on this form is accurate and complete to the best of my knowledge. I also commit to promptly notifying Wanderlust Pole Studio Pty Ltd of any changes that might occur.

I have carefully read, comprehended, and accepted the Regulations, Terms and Conditions outlined within the Wanderlust Pole Studio Pty Ltd Participation Waiver, you and your child have agreed to via our Online Booking system Wellness Living.
