Club registration for the 2023 membership year will have 2 parts.
1) Register with USA Swimming - (Pay the USA Swimming portion of the fees directly to USA Swimming at time of registration.)
2) Submit Club Charter Information (via this form) to MN Swimming & Pay LSC portion of the club registration fee - (MN Swimming will bill the team via ACH the LSC portion of the club membership fee based on date completed form is submitted. (If your club does NOT have ACH, you will be notified of the fee due and a check must be submitted.))
“Club” is defined as a group with athletes and coaches. Insurance Certificates will be available via the USA Swimming Website.
Regular membership is for one calendar year expiring on 12/31.
Seasonal membership is for a specified 5-month period as designated yearly. (For MN Swimming this is designated as 4/4-8/31 each year.)
If any positions on your club application change during the membership year, submit an update.Please keep all addresses/phone numbers and email addresses current. Again, contact MNSI with changes. Every position on this form must be completed & all fields for that position are required.
Notify club athlete and Safe Sport Athlete Liaisons that they have been chosen and remind them of their responsibilities.
