This form is a consent for Testing at Midwest Dizziness and Balance institute (MDBI).  Midwest Dizziness and Balance Institute (MDBI), will be performing a comprehensive set of diagnostic tests in order to assess the inner ear vestibular system. The tests I will undergo are proven to evaluate the inner ear to determine vestibular function while at rest and while moving. By initialing each item below, I am giving my permission for the providers of MDBI to administer all testing to me.
