This form is designed to gain an understanding for the need for food for students Pre-K through 12th grade.  This would be once a week on Thursdays until April 30th.  Delivery will take place at 2 pm.
They will be dropped off and left at the front door of the address you provide.  
Food will vary depending on availability.    
This is completely confidential and only Mrs. Freeman will see the results.  
Name and address are needed in order to accurately deliver food boxes and to make sure each family has taken the survey.
