RELEASE AND WAIVER: I/we realize that participation in dance classes and activities could result in some possible personal injury. Despite precautions being taken by Dance tHerAPPY, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Dance tHerAPPY. I/we will not hold Dance tHerAPPY or Zumba, liable for any personal injury or any personal property damage/loss, which may occur on the premises before, during or after classes.
 I/we acknowledge the inherent risks involved in participating in Zumba classes and utilizing the Dance tHerAPPY facility, including but not limited to injuries and accidents. I assume full responsibility for any such risks and release Dance tHerAPPY Facility, its owners, instructors, affiliates, and employees from any liability for injury, illness, or damage incurred during my participation. I agree to indemnify Dance tHerAPPY Facility for any claims or losses arising from my participation. I also grant permission for Dance tHerAPPY Facility to use any photographs or videos taken for promotional purposes. I understand that I am responsible for the security of my personal belongings, and I agree that this waiver is governed by the laws of Louisiana, with any disputes subject to the jurisdiction of Louisiana courts. I have read and understood this waiver and voluntarily sign it. This waiver agreement shall remain in effect until December 31, 2023, from the date of signing.
