If paying as a check, please send to 2560 Campus Rd.  George Hall 346 Honolulu, HI 96822. You can also set up meeting times with our Director of Finance, Quinn Sumida by emailing us at timsa.uhm@gmail.com. If using Venmo, please pay as: [TIMSA Fee] FirstName LastName.

**The deadline to complete this form and send in membership fees is Friday, January 27, 2023 at 11:59pm HST. Failure to do one or both of the items required by the deadline will result in termination of membership.

Cancellation Policy: You must notify TIMSA three days (72 hours) in advance if you CAN NOT attend an event.  Also, if it is an event that TIMSA is paying for or is partially paying for, you may be held responsible financially to cover costs. If the cancellation policy is violated twice, the TIMSA Executive Board will be taking away a credit (from your Active Status) of the board's choice.
