Student Housing Questionnaire
Student Housing Questionnaire
Please enter in the information below. If you have multiple children enrolled at Minot Public Schools, please enter the YOUNGEST child enrolled for the first entry.

The answers you provide will help the district determine what services you or your child may be able to receive under the McKinney-Vento Act. The Minot Public Schools Students In Transition Program (children and youth who are not in permanent housing) is jointly funded through the McKinney-Vento grant and Title I Part A grant.

The required housing questionnaire will be administered annually in the fall, in order that the district may address student needs. It should also be completed if your family's living arrangement changes during the year.

Thank you for taking the time to indicate where your child(ren) are currently living.
