In consideration of participation, I agree that:
a. We agree to assume all the risks and responsibilities of participating.
b. MCS cannot be held liable for any injury that may occur.
c. My student’s health does not preclude or restrict my student’s participation.
d. I have adequate health and hospitalization insurance and/or accept the financial responsibility for treatment if an injury does occur.
e. MCS has permission to authorize emergency medical treatment for my student.
f. MCS has no responsibility for any injury that might occur in connection with that treatment.
g. I will release, waive, and forever discharge any and all claims against MCS for any injury to me or damage to my property resulting from the negligence of MCS or anyone else involved.
h. We will not sue MCS, or seek any money from it or a judgment against it, for any injury to me or damage to my property resulting from the negligence of MCS or anyone else involved.
