This request form is to be completed by the volunteer/community coach/TMU student for a long-term badge.  

Volunteers: A long-term volunteer is someone who volunteers at the school on a regular basis throughout the year (i.e., Room Parent, Backpack Buddies, overnight chaperone for a field trip, etc.)

IMPORTANT: A new badge must be requested at the beginning of each school year after you have completed the Mandated Reporting Module (only) on the Compliance Director for that new school year.
