Remote Learning 2.0 Lunch Pick-Up Form
Please complete this form if you would like to pick-up a lunch kit at your school building on Monday of each week. The lunch kit will include enough food for lunches on school days for the week. Pickup times are listed below:
Tippecanoe Middle School - 11:30 @ Side Entrance Near Bus Garage
Tippecanoe High School - 12:00 @ Front Athletic Entrance

PLEASE NOTE:
If you need multiple lunch kits for multiple children on remote instruction, please complete a separate form for each child. This form must be completed by noon on Friday in order to be included in pick-up for the following week.

You will only need to complete this form ONCE per student as it will remain active as long as your building remains on Remote Learning 2.0.  If you no longer wish to receive lunches and would like to be removed from the list please email:  khagen@tippcity.k12.oh.us
