Student Teacher/Clinical/Field Experience Form
Please complete this form ANY TIME you are bringing an educator-in-training into your classroom for any type of formal experience (student teaching, clinical experience, field experience, job shadowing, etc..). It's incredibly important for building security reasons that we have the following information about the adults who are in our building and working with our students. In addition, we can provide a better experience for them if we are prepared for their arrival.
