Willow Street UCC Preschool Form I, the parent or legal guardian of the student, (hereinafter referred to as “MINOR”) hereby acknowledge that said ninor is presently under my care and custody, I hereby give said minor permission to go to and participate in activities with the Willow Street United Church of Christ (hereinafter referred to as “church”) during the camp. In the event there arises and emergency necessitating medical or surgical attention, I hereby consent and give my permission to the church, its employees, its representatives or trip leaders to make decisions to perform such medical treatments and/or surgery upon such minor which may in their sole discretion be necessary and proper under the circumstances. I, the undersigned parent and/or guardian of said minor do release, adequate, discharge to hold harmless the church, its employees, its representatives, or trip leaders from any and all actions, damages and or liabilities arising out of any accident or sickness, or treatment thereof, incurred by said minor during activities with the school.
