I agree that I will self-evaluate the day of the Friends of Chamber Music event I plan to attend, and will not come if I have any of the following symptoms consistent with COVID-19:
- Fever exceeding 100.4 F
- Headache
- Sore throat
- Dry cough
- Sudden inability to taste and smell
- Shortness of breath
- Ear aches
- Body aches
- Diarrhea
- Fatigue
- Vomiting
- Abdominal pain
I agree that if I am exposed to anyone with COVID-19, that I will not attend any Friends of Chamber Music events for the following 14 days after such exposure.

THIS TICKET OR INVITATION IS A REVOCABLE LICENSE SUBJECT TO THE FOLLOWING TERMS AND CONDITIONS
RULES, INSTRUCTIONS AND DIRECTIONS: The holder of this ticket (“attendee”) is granted one admission (along with any authorized minor(s) accompanying attendee) to the event noted on this ticket (the “Event”) subject to the conditions set forth below, the rules and instructions of the Friends of Chamber Music and the Montview Presbyterian Church (“Montview”), and all posted and/or verbal directions and instructions (including any social distance requirements).
BY USING THIS TICKET, Attendee acknowledges and agrees that holder’s license to attend the Event and access any venue or property on which the event is held (the “Venue”) is conditioned on and at all times subject to these terms and conditions. Failure to abide by such rules, instructions and directions may result in dismissal from the Event or Venue.
MASK POLICY: A suitable face mask is required at the Venue at all times before, during, and after the Event.
COVID-19 WARNING: An inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. According to the Centers for Disease Control and Prevention, senior citizens and attendees with underlying medical conditions are especially vulnerable. There may be additional risks that are unknown.
ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby voluntarily choose to accept all risk, for myself and any accompanying minor, of being exposed to, contracting, and/or spreading COVID-19, as well as all risks of property loss, personal injury, and all other hazards related to the Event or presence at the Venue, whether occurring prior to, during or after the Event, so that I may be able to attend the Event in person.
WAIVER AND RELEASE OF LIABILITY; INDEMNIFICATION: In exchange for admission to the Montview premises, Attendee, on behalf Attendee and any accompanying minor, hereby releases, discharges, and agrees to hold harmless (a) Friends of Chamber Music, Montview, and their respective affiliates, directors, officers, volunteers, employees, agents, contractors and representatives; (b) all Event attendees; (c) other event artists/performers; (d) Montview and its staff, sponsors, advertisers, Event promoters, and vendors; and (e) the successor and assigns of each of the foregoing (each considered one of the “Releasees” herein), to the fullest extent permitted by law, from any and all liability, claims, demands, losses, and damages, which Attendee and/or any minor accompanying Attendee has or may have for any claims or causes of action arising out of or relating in any manner to (i) Attendee’s and/or an accompanying minor’s presence on the Montview premises; (ii) attendance at, observation of and/or participation in any event at the Montview premises; (iii) any other related activities; or (iv) exposure to or contracting of COVID-19 or any other disease or illness, regardless of the Releasee’s preparation, or the public health measures put in place (or lack thereof) by any Releasee. In further exchange for admission to the Montview premises, Attendee agrees to indemnify, defend and hold harmless any and all Releasees from and against any and all demands, suits, claims, damages, losses, liabilities, costs and expenses (including reasonable attorneys’ fees and expenses), including but not limited to, any claims by or on behalf of Attendee or any accompanying minor, arising out of or relating in any manner to (i) Attendee’s and/or an accompanying minor’s presence on the Montview premises; (ii) attendance at, observation of, and/or participation in any event at the Montview premises; or any related activities, (iii) Attendee’s and/or an accompanying minor’s acts or omissions; or (iv) exposure to or contracting of COVID-19 or any other disease or illness, regardless of the Releasee’s preparation, or the public health measures put in place (or lack thereof) by Releasee. In exchange for admission to the Montview premises, Attendee covenants not to sue (or assist any person in suing) any Releasee for any matter related to the above assumptions, releases, indemnifications or agreements.
MINOR ATTENDEES: If Attendee is accompanied by any minor to the Montview premises, by allowing the minor(s) to accompany attendee, Attendee makes and agrees to all the foregoing assumptions, releases, indemnifications and agreements on behalf of such minor(s). If Attendee does not wish to or is not authorized to make such assumptions, releases, indemnifications and other agreements on behalf of the accompanied minor(s), Attendee should not allow the minor to enter the Montview premises.
