All PT Sessions must be paid for in advance. Sessions will not be confirmed until payment has been received.
I understand that by completing payment and agreeing to this contract I have 60 days to use my block of 4 PT Sessions. 
I understand that it is my responsibility to fit in my block of 4 PT sessions within a 60 day period.
If you are unable to continue your Personal Training sessions due to medical reasons, a refund may be available. You will need to provide reasonable evidence explaining that you are unable to continue for medical reasons. Refunds cannot be provided under any other circumstances.
You must give 24 hours notice to cancel a session, failure to do this will result in the loss of the session at the full rate.
If you’re late for your session, the session will not be extended and will end at the appointed time.
If you’re more than 20 minutes late, the session may be cancelled, and you will be charged for the full session.
All clients must complete a PAR-Q & Liability Waiver before commencing any exercise programme.
LM FIT has the right to terminate an appointment immediately if the client demonstrates inappropriate or unacceptable behaviour.
