4-H Record Book and Permanent Record Form Order
Thank you for your interest in ordering 4-H Record Book and Permanent Record Forms. A 4-H staff member will confirm that your order has been filled soon. You can pick up the forms Monday - Friday between 8:00am-4:30pm. A note that the office is closed on legal holidays. Call 608-224-3700 if you have any questions about office hours. 

A note that youth can complete the project record book forms on the computer and print the forms on colored (or white*) paper. If youth are interested in this option, the club can request sheets of plain colored paper. *Colored paper makes it easier to evaluate project record books and is encouraged, but colored paper is not required.
