This platform exists to connect the community directly with CSD’s Equity Department in the Office of the Associate Superintendent. This form is for the purposes of record keeping in all equity concerns and potential incidents of bias, discrimination, harassment, or sexual violence. This platform is anonymous to protect anyone who may fear reprisal or doesn't wish to share their personal information.

Please keep in mind that there is VERY little concrete change we can make with a complaint lodged anonymously. The best way for us to be able to investigate and address your concern, is, for you to leave this form and select the option that provides your personal details and contact information.
