Parent Selection Survey Second Trimester
This survey is designed to collect information that will allow Grass Lake School  to make educated decisions on the manner in which we open school for the second trimester beginning November 12.  Please answer all the questions and feel free to provide other information in the final question designated "other information" you want to provide.  As it stands right now, ISBE Guidelines state that masks will be mandatory for all Students, Teachers and Staff, and social distancing will be required.  In addition, any student showing ANY COVID-19 symptom will be sent home with all of their siblings until they have a negative COVID-19 test, or a doctor's note stating they have been seen and the symptom is caused by another issue and not COVID-19, and must be symptom free for 24 hours without the aide of fever suppressing medications.  Parents will also be required to daily self certify their students are healthy and COVID-19 free before 7am each school day.   Please note that Public Schools are governed by the State of Illinois and specifically the Illinois State Board of Education (ISBE).

I would be willing to send my child to school for in person learning under the following conditions:  Students and staff are required to wear masks and social distance 6 feet.   In addition, any student showing ANY COVID-19 symptom will be sent home with all of their siblings until they have a negative COVID-19 test, or a doctor's note stating they have been seen and the symptom is caused by another issue and not COVID-19, and must be symptom free for 24 hours without the aide of fever suppressing medications.  Students will be given breaks for hand washing, time with the mask off, and time to move and play (outside if possible). At this point the school day will be 7:45am - 1:45pm and will include Lunch. PE, Music, and Art will be provided remotely. .  One alternative being considered to allow maximum use of school space for social distancing and relocating students would be 7:45am to 12:45pm with sack lunch being sent home. These times may change depending on the responses from this survey *
I want to enroll my child/children in full remote learning for the second trimester.  Remote learning would be similar to what you were experiencing at the end of this past school year.  This remote learning would require students to complete a specific number of hours per day of online/remote work and complete assignments issued that would count toward a student's grade. *
