This survey is meant to gather your input regarding the reopening of MCS in September. Our reopening plan, developed by a committee of community/school stakeholders, is based largely on the guidance and requirements we have received from the New York State Education Department and Governor Cuomo. We will, however, do our best to meet the needs of our families and students. With that in mind, please COMPLETE A SURVEY FOR EACH of your children who attend MCS. Please submit your response(s) by Friday, August 7th. Thank you for your input.

Please indicate your transportation plans below. It is important that you are able to commit to the selection below except for in emergency situations. Since social distancing is required on buses, we are very limited in the number of students we can fit on each bus.
