 Food Security Prevalence and Perceptions on College Campuses Informed Consent
 
INTRODUCTION
You are invited to be in a research study investigating food access among different communities on college campuses. This study is being conducted by Dr. Emily Heying (College of Saint Benedict) and Dr. Zubaida Qamar (San Francisco State University).

The inclusion criteria for this study are:
18 years of age or older
Employed as a staff or faculty member at an institution of higher education (college campus) in the United States
Reside in the United States

The exclusion criteria for this study are:
Undergraduate students

We ask that you read this form and ask any questions you may have before agreeing to be in the study.

BACKGROUND and PROCEDURES
The purpose of this study is to investigate two different objectives:
1. To better understand food access of employees in higher education
2. To investigate faculty and staff perceptions of student food insecurity or lack of access to food on college campuses.

If you agree to be in this study, we ask you to complete a survey about your knowledge and awareness of food security at your institution. There will also be questions regarding your own access to food, your demographics, and means of employment. The survey will take approximately 10 minutes to complete.

RISKS/BENEFITS
This study presents minimal risk to participants. Some of the questions may bring uncomfortable feelings and slight stress. One benefit of participation is that upon survey completion, you can choose to provide your email address in a separate form for a chance to win one of two $25 VISA gift cards. An additional benefit is that your participation will help the researchers better understand the perceptions of food security on college campuses, which are important for designing interventions to support students.

CONFIDENTIALITY
The data collected with this study will be kept private. Your name will not be collected or associated with any information you provide.  Research records will be kept in a password secured document and only the researchers will have access to the records. In any reports, publications, or public presentations, no information will be included that would make it possible to identify a participant.

VOLUNTARY NATURE OF THE STUDY
Your participation in this research study is completely voluntary. You may stop participating at any time without penalty or costs of any kind. If you choose to stop participation, your data will not be recorded or included in the publication of the study. Your decision whether or not to participate will not affect your current or future relations with the researchers, College of Saint Benedict or Saint John’s University, or San Francisco State University.  

FUNDING SOURCES
The researchers are self-funding the incentives for this study.
