MADISON AVENUE DANCE- PARTY RENTAL FORM
HERE'S WHAT IS INCLUDED:

- Half your party time spent in the studio, second half spent in the lobby for cake and gifts!
- Tables and chairs (two 6' rectangle tables).
- A fabulous dance instructor in your very own private studio.
- Fun dance choreography to the song of your choice.
- Plenty of time for games and free play with instructor supervision.
- Optional 'party upgrades'...see below.

OTHER INFORMATION:

- Feel free to bring your own décor, food and treats (No décor can be taped to the walls. No food or beverages are allowed in the studio space, must be kept in the lobby. No alcohol).
- Renters are responsible for a general clean up of the space before leaving.
- Studio will be open 15 minutes before and after the scheduled party time to allow for setup and clean up. Additional set up time to decorate can be reserved for $25/hour as long as our schedule allows. Please note this in the form below.
- No outdoor footwear is allowed in the studio space.
- Once you submit this form, you will receive a confirmation email and e-invoice.
- A $50 non-refundable deposit is required to reserve your date (you will receive an e-invoice). Remaining balance is due the day of the party (you will receive an e-invoice for the balance).

I assume full and complete responsibility and liability for all guests and participants at the party and/or space rental. I agree to reimburse MADS for any and all damages that may occur as a result of this rental.
