If your child was NOT baptized at St. Thomas Becket, you must submit a NEWLY DATED copy of your child's Baptismal certificate by December 1, 2021. (A copy of the original is not acceptable under Canon Law). The certificate may be obtained by calling or writing the Church of Baptism. Please ask that it be sent to St. Thomas Becket prior to December 1, 2021.

If your child was Baptized at St. Thomas Becket, you DO NOT need to contact the church office to request a Baptismal certificate. Submitting this form is sufficient IF your child was Baptized at St. Thomas Becket.

Providing a record of Baptism is a common function of churches. If you have a special situation that makes this request difficult to complete, email Dez (dezs@stbeagan.org) as soon as you encounter an issue and no later than December 1.
