Re-Opening Survey for 6th, 7th, and 8th Grade

Based on new guidance from the state, we are going to bring back our 5th graders on April 5th and our 6th, 7th, and 8th grade scholars on April 26th. The logistics of this re-opening plan will be heavily based on family input and the results of this survey. It is required you fill out the following information to help us move forward with our re-opening plan.
