THE PURPOSE OF THIS FORM IS TO DETERMINE WHICH STUDENTS REQUIRE TRANSPORTATION FOR THE 2024-2025 (NEXT) SCHOOL YEAR.
ALL FAMILIES ARE REQUIRED TO COMPLETE THIS TRANSPORTATION FORM WITH THE EXCEPTION OF:
1. PRESCHOOL - WE DO NOT PROVIDE
PRESCHOOL TRANSPORTATION UNLESS REQUIRED BY A STUDENT'S IEP. IN THAT CASE, YOU WOULD NEED TO CONTACT THE SPECIAL SERVICES OFFICE AT (978)582-4100 X5213.
2. SPECIAL SERVICES - THIS FORM IS
ONLY FOR REGULAR TRANSPORTATION.
IF YOUR CHILD IS A SPECIAL SERVICES STUDENT AND REQUIRES VAN TRANSPORTATION PER THEIR IEP, PLEASE CONTACT THE SPECIAL SERVICES OFFICE AT (978)582-4100 X5213.
3. SCHOOL CHOICE- OUR DISTRICT DOES NOT TRANSPORT SCHOOL CHOICE STUDENTS.
PLEASE FILL OUT A SEPARATE FORM FOR EACH STUDENT IN YOUR FAMILY.
THE DEADLINE DATE TO SUBMIT THIS FORM IS MAY 1, 2024. FAILURE TO SUBMIT THIS TRANSPORTATION FORM BY THE DEADLINE WILL RESULT IN THE STUDENT BEING PLACED ON A WAITING LIST IN ORDER OF RECEIPT FOR UP TO 60 DAYS.
