Teaching & Learning Agreement
Thank you for making the decision to train with us. Our aim is to help you to get the best out of the training and enable you to achieve a successful assessment outcome.

It is exciting to have the opportunity to work with other professionals and you, like them, will want to get as much benefit from every session over the coming two days.

To make sure that you and your fellow learners and the tutor get the most from the training, please read and digest the tutor and learner ‘working guidelines’ below and when you recognise and understand each one, please sign and date the Teaching and Learning Agreement as indicated.

 

As your Tutor, I will:


Ensure that I work within our agreed timetable and start and finish the day on time.

Inform you of the purpose and process of each session

Use training resources that allow you to maximise your learning

Give you some opportunities to make comments and questions in relation to the learning.

Invite you to comment and contribute to the training.

Inform you of any health and safety issues in relation to the venue.

Provide individual and confidential advice and guidance

Adhere to our Equal Opportunities Policy.

Attend all sessions.

Attend sessions on time.

Bring all necessary paperwork, activities and home based work as requested.

Actively participate in the training sessions with my group and tutor.

Take responsibility for my own learning by discussing my progress with the tutor.

Please answer as honestly as possible the health related questions below. If you have anything additional to add there will be space for it under. If there are any queries regarding your health and exercise the tutor will be in touch to discuss them or will discuss them on the day.
Has your doctor ever said that you have a heart condition and that you should only do physical exercise recommended by the doctor? *
Do you ever feel pain in your chest when doing physical activity? *
Have you ever had chest pain when you are not doing physical activity? *
Do you ever feel faint or have spells of feeling dizzy? *
Do you have a joint problem that is made worse by exercise? *
Have you ever been told that you have high blood pressure? *
Are you currently taking any medication that the instructor should be made aware? *
Are you pregnant, or have you had a baby in the past 6 months? *
Do you know of any other reason why you should not participate in physical activity/exercise? *

If you have answered Yes to one or more questions:

Talk with your doctor by phone or in person before you become much more physically active or before you have a fitness assessment. Tell your doctor about the Par-Q Pre-Exercise Questionnaire and to which question you answered Yes.

You may be able to do any activity you want as long as you start slowly and build up gradually. Or, you may need to restrict your activities to those that are safe for you. Talk with your doctor about the kinds of activities you wish to participate in and follow professional medical advice.

If you have answered No to all questions:

You can be reasonably sure that you can start becoming physically active and taking part in a suitable exercise programme. Begin slowly and build up gradually. This is the easiest way to exercise.

Delay becoming more active if you feel unwell because of a temporary illness such as a cold or a fever – wait till you are better.

Please note: if your health changes so that you answer YES to any of the above questions, inform your fitness or health professional immediately.   Ask whether you should change your physical activity or plan.
