Please complete the following form to request a Verification of Enrollment (VOE) from Oak Ridge High School. VOEs are only valid for 30 days from the date they are issued by the school.

Please remember that in order to receive a VOE from the school, you must:
- be a current student between the ages of 15 and 18
- have met the 90 percent attendance requirement for the semester preceding the date of application
- Not have any financial holds at school (unpaid fees, lost textbooks, etc)
**If you are on the denial of credit list, we will not be able to issue you a VOE until you make up your lost time.**

Senior Campus Students (Grades: 10, 11, and 12): 
VOEs will be available for pickup at the front office of the Senior Campus for students who are in the 10th, 11th or 12th grade on Tuesdays and Thursdays from 10am - 1pm. If you submit your request Wednesday through Sunday, your VOE will be available for pickup on Tuesday, and if you submit your request Monday or Tuesday, your VOE will be available for pickup on Thursday.
