I understand that I am applying to work extra hours in another Harc program. My application to work extra hours is subject to an application and approval process and cannot interfere with my regular hours that I am scheduled to work in my regular position.

If my application is approved, I agree to the following conditions:

I must complete site and participant specific training within 1 month of being accepted to work extra hours.

I will be offered hours as they are available. If I accept the hours, I will be assigned to the shift/s and therefore expected to work. In the event that I find myself unable to work due to unforeseen circumstances, I will speak directly to the extra hours supervisor in the appropriate amount of time as stated in the policies and procedures for the department. I am not guaranteed any hours.

I will inform my extra hours supervisor of my availability to work and any changes in that availability in a timely manner.

I am required to follow all of HARC’s policies and procedures including the notification and other requirements of HARC’s attendance procedures.

As an extra hours staff, I will be subject to progressive discipline, including discipline for indicating availability to work, accepting assignments, and then calling out.

I will maintain my required in-service training and certifications for my regular position and for the program(s) where I am working extra hours in.

As an extra hours staff, I understand that I am not eligible for any additional benefits, including, but not limited to paid time off (PTO) health or dental insurance coverage, life insurance or long term disability insurance that I am not already entitled to.

I also understand that although I am ineligible for other benefits, I may be eligible for Paid Sick Leave if I meet the requirements for this benefit. I understand that I must work at least 680 hours in each calendar year and meet other requirements, by law, in order to be eligible for Paid Sick Leave.

If I am an extra hours staff member in any of HARC’s Residential Programs, Recreation/Respite, Camp Bulova, I must be available to work weekends, if offered.

For all other programs, I must be available to work the minimum hours as posted.

I understand that I cannot accept extra hours that interfere with the hours of my regular work assignment.

I understand that I must comply with all Harc and departmental policies and procedures including but not limited to Harc Procedure Section XIII. Maximum, Consecutive and Extra Hours.

I understand that I will be assigned no more than 2 extra hours programs, not including my regular assignment.

I understand that if I am suspended, put on administrative leave, etc., I am not permitted to work an extra hours assignment during that period.

I understand that I will be removed from the extra hours list for:

- Failure to complete my site and participant specific training within 1 month of assignment
- Excessive cancellation of shifts after accepting work; (i.e. Cancellation of at least 2 shifts per month, three months in a row).
- Excessive refusal to accept shifts offered; not being available to work for a period exceeding 3 months, if offered shifts, (unless I have made prior arrangements with my extra hours supervisor).
- Failing to obtain or maintain all required in-service trainings and certifications
- Failing to follow HARC, Residential, Day Programs, or Community Services policies and procedures
