USE THIS FORM TO REPORT AND REFLECT on hours of service you completed in the Fall 2020 semester (service hours prior to the start of the Fall 2020 semester may not be reported). You will receive one co-curricular credit for every two hours of service rendered in a church, community or campus context (up to 20 hours).

PLEASE NOTE: Co-curricular credit is not offered for paid workplace experience. This form is only for reporting voluntary service hours.

DEADLINE: This form must be submitted by 5 p.m. on Friday, November 13, 2020. Credit will be issued once the Reference you list in Section 3 has reviewed your reflections and approved your service hours.
