In order to best manage any medical emergency that may occur during Sailing School, it is important that the staff be aware of all important or special medical conditions of the students. Please complete the form prior to your children starting Sailing School.
If your child is returning and have filled out a medical form in the past, you do not need to submit one this year UNLESS you have new information to tell us. This could be either new health information, or new emergency contact information.
