The information you submit on this form will be EXACTLY what is provided to our announcer on Senior Night.  We want to honor every senior and family as best we can as you all have given so much to this program.  We can only do this if we have the most accurate information from you.  Please don't hesitate to leave a note for us and we will make every effort to thoroughly address any requests.  Please be very detailed about what you would like read and how you would like it read.  If a name is not common, please provide us with a phonetic spelling or some way of knowing how it should be pronounced.  Please list whomever you wish to appear on the field with your senior, keeping in mind our time and space constraints.  If we have an issue with the number of people being requested, we will contact you.
Graduating Student's FULL NAME as you would like it to be read by the announcer (Shortened, alternative, or nick-names are completely fine with us!) *
Please list anyone who will be escorting the graduating senior during Senior Night.  Please include the person's FULL NAME exactly as you would like it to be read by the announcer, as well as his/her relationship to the graduating senior.
