  • PARTIAL REPORTING WILL NOT BE ACCEPTED.
  • PRODUCERS SHOULD USE THE SAME PRODUCER'S NAME AND EVENT NAME THAT WAS PROVIDED ON THE REQUEST FOR BONUS QUALIFIER APPROVAL.
  • SINGLE APPROVED SHOWS NEED TO COMPLETE SEPERATE FORMS FOR EACH SHOW DAY.
  • ALL QUALIFIERS WILL OWE $1 PER RUN OR A MINIMUM PAYMENT OF $50 IF THE TOTAL NUMBER OF RUNS IS LESS THAN 50 ENTRIES.
  • IT IS MANDATORY TO FULLY COMPLETE AND SUBMIT THIS FORM OR THE PRINTABLE REPORTING FORM WITHIN 10 DAYS OF YOUR SHOW.
  • IT IS MADATORY TO MAIL YOUR PAYMENT WITHIN 10 DAYS OF YOUR SHOWS.
  • IT IS MANDATORY TO INCLUDE A $50 LATE FEE WITH YOUR PAYMENT IF IT IS POSTMARKED MORE THAN 10 DAYS LATER THAN THE CONCULISION OF YOUR EVENT. 
  • IT IS MANDATORY TO PROVIDE YOUR EVENT'S FULL RESULTS BY MAIL OR EMAIL TO ONE OF THE ADDRESSES BELOW WITHIN 10 DAYS OF YOUR EVENT.

What is the total number entries of the show you are reporting? SINGLE approval events need to report each day separately. SERIES approved events need to report the total number of runs made throughout your entire series. If you are reporting for a DOUBLE QUALIFER, please type the number of entries followed by the word "DOUBLE".EXAMPLE 162, SINGLE *
Select the number of qualifiers that should be awarded for the series or single date you are reporting for. 

If your event was within 20 entries of the next level, you are allowed to pay for those additional runs to bump your qualifier level up. Please select the option of the number of qualifiers you are bumping up to. This will only be honored if your payment reflects this. 

If you are reporting a DOUBLE QUALIFIER, please choose the appropriate number of qualifiers to be awarded, (take your number of Open entries x2).
