The sleep history questionnaire should take you approximately 20-30 minutes to complete. If you'd like your progress saved as you go, be sure to log in to your Google account. As always, all information you provide to The Baby Sleep Site® will be kept strictly confidential.

Once you complete the assessment, your responses will be sent to a sleep consultant, who will make over 50 decisions and craft your Personalized Sleep Plan®. You can expect to receive your Personalized Sleep Plan® within two business days, on average, depending on demand. If you are in a hurry, we offer expedited processing for a one-time, non-refundable fee of $39. Please contact customer service once you complete the assessment for more information.
