TERMS OF REGISTRATION: As the parent/guardian of the above-referenced child, I hereby register and give my child permission to fully participate in the Winter Birds and Bird House Making Workshop listed above that is offered by the Eastern Queens Alliance Idlewild Environmental Science Learning Center held on Saturday, January 30th, 2023. I understand that for outdoor Field Trips and any indoor workshops in the Idlewild Environmental Science Center my child must wear a mask for all sessions, follow all social-distance requirements, and be dressed appropriately for the season. I also understand that during certain seasons my child may need to use insect repellent. I hereby absolve and hold harmless Eastern Queens Alliance, Inc., its subsidiaries and affiliates, directors, employees and officers from any and all liability for any injuries or unforeseen negative impacts incurred by my child or ward as a result of his/her participation in the full scope of components of Eco-Movie/workshops, fieldtrips and any online Science-in-the-Park Program offerings.
