◼️訂購方式及流程

填寫此表格以下各部份即可完成訂購程序：
1. 選購喜愛的紀念品及選擇數量
2. 選擇付款方式 (劃線支票 / 銀行存款 / 現金)
3. 選擇送貨或親臨秘書處取貨
4. 填寫聯絡資料
5. 閱讀及同意條款及細則，以及個人資料收集聲明
6. 提交訂購表格

秘書處職員於三個工作天內發送「訂單摘要」電郵。請按電郵指示完成付款，秘書處職員將於收到款項後發出「訂單確認」電郵，並列明送貨或取貨安排。

閣下亦可選擇於訂購服務時間親臨協會秘書處選購紀念品，並以現金付款。

A fine collection of souvenirs of the Hong Kong Sports Association for the Physically Disabled (the Association) is available for sale.  You are welcome to submit this Order Form or visit the Secretariat during ordering service hours to purchase the souvenirs.  Please read Purchase Notice https://www.hksapd.org/home/content.php?id=3108 before ordering.

◼️Purchase Methods and Process

In this Form, you are going to
1. Select favourite souvenirs and quantity
2. Select payment method (Crossed Cheque / Bank Deposit / Cash)
3. Select delivery or pick-up at the Secretariat
4. Enter contact information
5. Read and agree to the Terms and Conditions, and the Personal Information Collection Statement
6. Submit this Form

The Secretariat staff will issue the Order Summary email within three working days.  Please make payment as indicated in the email.  The Order Confirmation email will be issued with delivery or pick-up details stated after receiving the payment.

If you wish to order in person, you are welcome to visit the Secretariat during ordering service hours and make purchase by cash.
