Close Contact Request for Remote Conferencing
Your child may be eligible for remote conferencing if he/she has been identified as a close contact of someone who has COVID 19.  If you are choosing to quarantine your child, please complete the following form.  Within 24 hours of receiving this form, an email will be sent out via ParentLink with remote conferencing instructions.  You must submit a separate form for each child that is going to be in quarantine.  If your child is fully vaccinated for COVID 19, he/she is not considered a close contact and does not qualify for remote conferencing.

Quarantine guidelines recommend that a close contact, who is unvaccinated, quarantine at home for 10 days following the last date of contact with a COVID 19 positive individual.  Close contacts may return to in-person instruction on day 7 if he/she receives a negative COVID 19 test on day 5 or later.
