I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. As the guardian, I am fully and personally responsible for my child's safety and actions while and during participation in the camp, and I recognize that she may be in any case be at risk of contracting COVID-19. By acknowledging below, I am not holding Forte responsible and will take all necessary precautions with keeping my child home if feeling ill. *
Forte Media Release 2020-2021. I give permission to Forte Performance to use my child’s photo and name in any publicity efforts associated with the Forte for 2020- 2021. I understand that this photo may be used on Forte's website, social media accounts, and any handouts associated with Forte. I understand that this permission may be revoked at any time in writing by sending an email to thefortefrontdesk@gmail.com. By electronically agreeing to this agreement, I agree to hold Forte Performance LLC harmless for any liability arising from participation. *
