This form is to indicate there was a change of leadership (Principal and/or CS Coordinator) at any point during which the school received a NMPED community school grant. 

Please have the new Principal and/or CS Coordinator fill out this form. This form must be filled out as a prerequisite before applying for a NMPED CS grant. 

This form is not an evaluation or garnering of grant points. This form is to provide PED with contact information and indications if further support is needed.
