I, the undersigned, understand and acknowledge that I may be engaging in physical activity and that the use of equipment and instruction could cause injury. I, the undersigned, acknowledge that I have voluntarily enrolled in this program and assume responsibility for all risks of injury resulting from my participation in activities of Dance and Bmore programs. I hold harmless Dance and Bmore and its instructors for any and all claims of liability caused by injury.
