I, the undersigned parent or legal guardian, authorize the below-named student (the “Student”) to participate in the Omaha Area Youth Orchestras program (“OAYO”) .I acknowledge there are risks in any youth program.

In consideration of the right of Student to participate in the activities offered by OAYO, I agree to release, hold harmless and indemnify OAYO, its directors, officers, employees, volunteers, and agents from any and all liability, claims and causes of action arising out of or in any way connected with Student’s participation. If I am signing on behalf of a minor, I further agree to hold harmless and indemnify OAYO, its directors, officers, employees, volunteers, and agents from all liability, claims and causes of action that the minor may have arising from the minor’s participation in the activities of OAYO.
I understand that although the Released Parties may be present at performances and other OAYO activities (the “Activities”), the Released Parties will not be responsible for supervision of the Student. I am solely responsible for supervising the Student in connection with the Activities. I and the Student shall comply with all applicable laws in connection with the Activities, including all laws concerning alcohol and minors.

I understand that the Student is responsible for his or her instrument and other personal property
brought to OAYO activities. I agree that if I need insurance to cover the instrument, I will arrange for
such insurance with my own insurance company. I hereby agree to indemnify, hold harmless and release
from liability OAYO and Released Parties for damage or loss to such instruments or personal property.

I agree that both I and Student will abide by all OAYO and Venue safety rules. I also agree to contact the
OAYO Executive Director or other OAYO staff immediately concerning any injury to student, damage to
student’s property, violation of OAYO policies, Venue’s rules, or violations of law. As a condition of utilizing the Strauss Performing Arts Center, OAYO is required to comply with the UNO Youth Activity Safety Policy and is required to provide copies to all staff, volunteers and parents or guardians.
