EAS2021 code of conduct violations report form

This form allows to report, in a fully anonymous manner, any violations of the code of conduct (https://eas.unige.ch/EAS2021/codeconduct.jsp) you were the recipient or witness of within EAS2021, including on the meeting platform, on the EAS2021 Slack space and in the Gather.town space. The information received will be accessible only to the following people: Sara Lucatello (INAF, EAS Vice-president), Inma Dominguez (University of Grenada, EAS secretary), Marc Audard (University of Geneva, EAS executive secretary), Leo Burtscher (Leiden observatory, EAS Sustainability Working Group co-chair), Jake Noel-Storr (Leiden, EAS Inclusion Working Group co-chair). The only thing that is collected besides the information you provide is the timestamp of the submission. All the information will treated with the utmost confidentiality.

Please provide a description of the code of conduct violation you were the recipient of or witnessed. Keep in mind that details such as where the event took place (On Air platform, Slack or Gather.town), are helpful to assess the situation.
