Washington Yu Ying Afternoon Carpool Pass Request Form
Please read the notes and directions before completing this form. It has changed from last year.

Notes:
-During afternoon dismissal, Yu Ying's driveway will be reserved for families picking up 3 or more students (regardless of age) as well as families with an infant under 6 months in the car or with Handicap needs.
-Families picking up fewer than 3 students should park legally in the neighborhood and walk onto campus.
-Once this form is completed and reviewed, families will receive a confirmation email and will be on an authorized carpool list.
-Those with handicap needs or other extenuating circumstances, must list at least 1 student. ("N/A" where otherwise applicable).
-We are not able to accommodate play dates or sports pick ups that happen twice a week or less. Instead, those should be requested via 'The Online Attendance Form' each day.

Directions:
1)For families who are carpooling with children outside of their family:
CHOOSE one family name for all the students to be called by during daily pick up-  This name will be used to call the entire group of students and will be used as the main contact person, regardless of the driver that day.   Please communicate the family name to all the students in your carpool group.
2) SELECT the carpool days - This will allow us to have the students ready on the correct days.
3) LIST remaining student information for each additional student

Please share why your family will need daily access to the carpool lane. Please bring in handicap sticker to the receptionist to receive the carpool pass.
