Parent Guardian Zoning Acknowledgement: Candidates may only tryout for cheerleader/mascot at the campus for which they are zoned. By entering your name below, you understand that it is your responsibility to ensure that your student is trying out for the campus for which the student is zoned.  If a transfer/move occurs after tryouts, the student forfeits his/her position on the current team and will not be placed on a team on the campus to which the student is transferring/moving. If student is found to not be zoned for the campus for which they are selected, the student will forfeit the position on the team. There are NO refunds or exchanges for any reason. This includes ordering incorrect items due to trying out at the incorrect campus. *

You will also need to provide copy of the front and back of the student's insurance card or $5 cash to purchase camp insurance. *
Parent/Guardian Conduct Acknowledgement: By entering your name below, you understand cheerleaders/mascots are considered athletes and will follow the same guidelines/rules as all other MISD athletes. You understand cheerleaders/mascots are considered athletes and will follow the MISD Student Code of Conduct, MISD Extra/Co-Curricular Contract, MISD Athletic Rules, MISD Cheerleading Guidelines and any additional campus guidelines, handbooks, or rules. You also understand that duties begin as soon as results are posted and do not end until the last day of school of the 20-21 school year. *
