Dear Parents & Students,

Thank you so much for taking our Food Service Survey! Nutri-Serve Food Management is the food service company. Nutri-Serve is looking to get some feedback on the menu to help us improve the program. We want to learn what menu items you like and what you want to see on the lunch menus!

Each student should complete this individually with their parent/guardian so we can get the most accurate feedback as possible! If you have more than one child in the district please fill out one survey per student.
